Social Prescribing Service

Hello, I’m Zainab, the Social Prescriber at E16 Health. My role is to support you with the social and emotional factors that can affect your health and overall wellbeing.

Many everyday challenges can have a real impact on how you feel, both physically and mentally. This may include loneliness or isolation, housing difficulties, financial worries or debt, or challenges managing your money.

If you are referred to me, you will have the opportunity to talk about what matters most to you. Together, we will develop a personalised plan to help you address these issues and take positive steps forward.

 

Who is this service for?

This service is for anyone aged 18+ who is registered at E16 Health and is experiencing non-medical issues affecting their wellbeing. This may include stress, loneliness, financial concerns, housing difficulties, or challenges accessing support services.

 

What is Social Prescribing?

Social prescribing supports people with non-medical needs that affect their health and wellbeing. This may include:

  • Benefits and financial support
  • Bereavement
  • Employment and volunteering
  • Health and wellbeing
  • Housing concerns
  • Loneliness and social isolation
  • Mental health (including anxiety, depression and stress)
  • Transport and mobility

Whether your concerns are short-term or long-term, the Social Prescriber is here to listen, help you identify your priorities, and connect you with the right support.

 

What the service offers

Support may include:

  • Developing a personalised support plan, from a single session or ongoing appointments
  • Providing information and guidance on available services
  • Referring you to relevant support services, where needed
  • Linking you with community groups and social activities
  • Using coaching and goal-setting techniques to support your goals and wellbeing

 

How to access the service

  • You must be aged 18 or over
  • Your GP may refer you if they feel you would benefit from support
  • You can also self-refer using the Joy link, or contact Reception or book through the Evergreen app.
  • Support is available via telephone or face-to-face appointments.

 

What happens after your referral to social prescribing?

After your referral is submitted, our Social Prescriber will contact you to arrange an initial 30-minute telephone appointment. During this conversation, we will discuss your needs and agree on the best way forward together.

Waiting times may vary, but we aim to contact you as soon as possible after receiving your referral.
